Dr. Phil Berry’s retirement effective June 30, 2018
Dear Patients,
It has been my pleasure to serve our community for the past 47 years as an orthopedic surgeon and physician. I have had the privilege of meeting so many people and developing lifelong friendships with many patients. But, now it is time for me to refer my patients to the other competent physicians at Oak Cliff Orthopaedic Associates dba Texas Orthopaedic Surgical Associates.
All your medical records are safely on file at OCOA/TOSA and remain confidential, as per federal law. If you decide to see a new physician outside of our practice, then you may follow the traditional routes to have your records transferred.
Over the past 5 decades, I have watched many families in the area grow and have shared heartache and joy with thousands of patients. I appreciate your loyalty throughout the years and will take the memories of all that I have experienced with me into my retirement.

We are moving to a new location
We are excited to relocate on Monday, March 30th to our new office located at 12230 Coit Road, Suite 130, Dallas, Texas 75251. Our phone number and fax numbers remain the same. Please inform your staff and patients we are still in the neighborhood, just down the block from Medical City Dallas Hospital.
In our efforts to better serve our patients, we have added more exam rooms and upgraded our X-ray technology. Our new location is on the first floor and no charge for parking.

As of Oct. 6, 2014, the Drug Enforcement Administration has elected to
change the status of all products that contain hydrocodone to a Schedule
II category
The new rules mean that drugs like Vicodin, Lortab, Norco and Vicoprofen and their generic equivalents will be subject to the same prescribing rules as painkillers like dilaudid and oxycodone.
Telephone refills will not be allowed. You will need to schedule an appointment to see your doctor before obtaining a new prescription. This will be required every time you need a refill of a pain medication with Hydrocodone, Vicodin, Norco and Vicoprofen. Your physician may require that you have drug testing, urine or saliva, and may also require you to sign a pain contract for ongoing narcotic use. You may also be referred out to a Pain Management Specialist.
This also means that phone-in orders and fax orders will no longer be allowed for those medications. Only hand-carried orders will be considered as valid prescriptions.
Any hydrocodone prescriptions written before Oct. 6 with refills will be allowed to be filled as usual. If written on or after Oct. 6, no refills will be allowed per normal DEA schedule II rules.
This change to the status of drugs containing hydrocodone is a national change and will affect every patient regardless of the pharmacy they use and state they live in.
